Perimenopause, Immune Changes and Your Skin
One of the more confusing changes I see women experience during perimenopause is suddenly becoming more reactive.
Your skin might become itchy, red or sensitive. An old skin condition might flare up again. You might notice rashes, flushing or reactions that seem to come from nowhere.
And understandably, you’re wondering — why now?
One piece of the puzzle we don’t talk about enough is the relationship between our reproductive hormones and our immune system.
Your hormones influence your immune system
Oestrogen and progesterone don’t just regulate periods and reproduction. They interact with immune cells and help influence inflammatory signalling throughout the body.
During perimenopause, oestrogen can fluctuate significantly while progesterone often becomes less consistent as ovulation becomes more irregular. As we move towards menopause, levels of both hormones eventually decline.
These changing hormonal signals can alter the way our immune system regulates inflammation.
Research suggests the menopausal transition is associated with changes in inflammatory signalling, including increases in some pro-inflammatory cytokines. These are chemical messengers used by the immune system to communicate and coordinate an inflammatory response.
And then there’s histamine
Hormonal changes can also interact with mast cells — immune cells involved in allergic and inflammatory responses.
Mast cells release histamine, and oestrogen can influence mast cell activity and histamine signalling. When hormones are fluctuating considerably, some women may find that symptoms associated with histamine become more noticeable.
Mast cells are found throughout our skin, so this may help explain why some women experience more itching, redness, flushing, sensitivity or reactive skin during perimenopause.
Your skin is part of your immune system too
We often think of skin as simply the outside covering of our body, but it is actually an active immune organ.
It contains immune cells that are constantly communicating with the rest of the body and responding to what is happening both internally and externally.
This is why I think it’s important to look beyond the skin when new symptoms appear.
Stress, poor sleep, gut health, blood sugar instability, nutrient deficiencies and hormonal fluctuations can all influence inflammatory and immune regulation.
Rather than simply trying to “boost” the immune system, I’m much more interested in helping it regulate appropriately.
That means looking for the individual factors that may be adding to your overall inflammatory load and supporting them one by one.
So if your skin suddenly seems much more reactive in your 40s, it may not be completely random.
Sometimes your changing skin is another clue that your hormones — and the immune system they communicate with — are going through a transition too.
